Your name, address, telephone number, GP details, past medical history and assessment results will be collected, as required, when you engage in our services. All of your data can be erased upon request at any time. For medical health record purposes, we shall securely store your archived data electronically for 10 years. 

Purpose of collecting data
We shall only collect your personal data in relation to your assessment and health profile. We shall never share your data with another party. Active Healthcare professional staff are the only people in our organisation that have access to personal data which is securely stored.
